Chief Operating Officer

Dr. Jokho Farah is a multi-award-winning physician with more than 15 years of experience spanning healthcare management, public health, and policy. She leverages data analytics and predictive modeling to design integrated care strategies that align services across the continuum for priority populations. Dr. Farah also advises federal, state, and local agencies on advancing health equity through policy, systems, and environmental change.

An ecosystem builder and community connector, Dr. Farah is known for fostering cross-sector partnerships that strengthen community health and advance equity. She made history as the first woman of color and first Somali American to lead the American Heart Association Twin Cities Board. She also served two terms as co-chair of the Minnesota Department of Health’s Health Equity Advisory Leadership Council.

Her leadership has earned widespread recognition, including the Stanford Social Innovation,  Health & Wellbeing Award from the Women’s Health Leadership TRUST, “40 Under 40” honors from the Minneapolis/St. Paul Business Journal, the American Heart Association’s Mission Impact Award, and the Minnesota Department of Health’s Lou Fuller Award for Distinguished Service in Eliminating Health Disparities—among many others celebrating her sustained commitment to advancing health equity.
