Case Management and Patient Assistance

Our Health Navigator Coordinators (HNCs) provide case management and help patients like you navigate the health care system. They are skilled practitioners who assist community members and patients in accessing OCHC’s services and also provide crucial follow-up.

They also participate in community events and work one-on-one with community members and patients through our Community Health Connection programs.
