What to Expect at Your First OCHC Appointment…

As a Federally Qualified Health Center (FQHC), we are committed to delivering high-quality, affordable healthcare to all individuals, regardless of their financial status, insurance coverage, or background.

Having a plan can help you get the most out of your visit.

Before Your Visit

  • Write down your top health concerns.
  • For first visits, list your medical history, including surgery dates.
  • Need an interpreter? We’re happy to provide one.
  • Complete any lab tests or procedures beforehand.
  • New patients: arrive 30 minutes early.
  • Returning patients: arrive 15 minutes early.

What to Bring

  • All medications, including herbs and vitamins.
  • Insurance card, photo ID, and copay.
  • Proof of income (last 30 days) for sliding fee discounts.
  • A family member or friend is welcome to join you. They can help take notes.
  • Not the guardian? Bring consent for minors or incapacitated adults.

During Your Visit

  • Ask questions, we’re here to help.
  • Set a health goal.
  • Be honest about your health

After your visit

  • Follow your care plan
  • Take your medication as directed
  • Contact us about any changes in your health

How much will it cost?

Co-payments are due at the time of service. Patients without health care coverage can apply for our sliding fee discount based on income and family size.

Open Cities may be able to identify additional funding support for our patients. Certain programs may help patients meet their financial responsibilities for the health care they receive.

Request My Records

To request a copy of your health records or to give permission for someone else to receive your records, print and fill out this form. Bring it in to the health center where you received services, and we will take care of your request. There is a cost for copies of health records, depending on number of pages. Return form to your health clinic in person.
