Plan Ahead for Your Visit

Before Your Visit

  • Write down your main health concerns.
  • For your first visit, bring your medical history and surgery dates.
  • Let us know if you need an interpreter.
  • Complete any lab tests or procedures before your appointment.
  • New patients: arrive 30 minutes early.
  • Returning patients: arrive 15 minutes early.

What to Bring

  • All medications, including herbs and vitamins.
  • Your insurance card, photo ID, and copay.
  • Proof of income from the last 30 days if applying for sliding fee discounts.
  • A family member or friend (optional) to help listen or take notes.
  • If you are not the guardian, bring consent for minors or incapacitated adults.

During Your Visit

  • Ask questions.
  • Set a health goal.
  • Be honest about your health.

 

Respectful Conduct Policy

Open Cities Health Center (OCHC) is committed to maintaining a safe and respectful environment for all patients, staff, and visitors. 

Patients are expected to treat staff and others with civility and respect. Rude, abusive, discriminatory, threatening, or disruptive behavior — including threats of violence — will not be tolerated.

OCHC reserves the right to discontinue care if a patient’s behavior compromises the safety or well-being of staff or others.

How much will it cost?

Co-payments are due at the time of service. Patients without health care coverage can apply for our sliding fee discount based on income and family size.

Open Cities may be able to identify additional funding support for our patients. Certain programs may help patients meet their financial responsibilities for the health care they receive.

Request My Records

To request a copy of your health records or to give permission for someone else to receive your records, print and fill out this form. Bring it in to the health center where you received services, and we will take care of your request. There is a cost for copies of health records, depending on number of pages. Return form to your health clinic in person.
