Get Coordinated Care with AccessPlus+
Let’s face it: Staying healthy isn’t always easy. At Open Cities Health Center we don’t want you to think you’re in this alone. That’s why, in addition to being a Health Care Home to all of our patients, we offer AccessPlus+, a program for patients whose path to health may benefit from a more coordinated approach.
The Community Health Connection
The Community Health Connection is OCHC’s outreach and education program.
Through the Community Health Connection program, we provide outreach, education and screenings across an array of health-related topics and diseases including:
- Breast and Cervical Cancer Screenings
- Nubian Moms program for African American mothers and pregnant women
- Dental Outreach and Education
- Flu Shot Clinics
- MNsure Assistance
