Health Navigator Coordinators

Our Health Navigator Coordinators (HNCs) provide case management and help patients like you navigate the health care system. They are skilled practitioners who assist community members and patients in accessing OCHC’s services and also provide crucial follow-up.

They also participate in community events and work one-on-one with community members and patients through our Community Health Connection programs.

Health Navigators provide:

  • Outreach, education, health screenings, support services, and home visits
  • MNsure enrollment and follow-up
  • Case management and follow-up
  • Tabling and support for community health fairs, including Black History Month and Hmong Health Fairs

Health Navigator Coordinators are an important part of our healthcare team. They help us ensure you get the care you need delivered in a relevant and personalized way that works for you. Our team works directly with MNsure to help facilitate an easy access and enrollment process for you.
