Quality Programs:
Open Cities Health Center conducts outreach to our patients to make sure they are up to date on their preventative health screenings, as well as calls patients to get appointments on the calendar to manage their chronic illnesses and give them the tools to take care of their health outside of clinic.
- We regularly follow-up with patients who have Diabetes and Hypertension and offer educational booklets that come equipped with a blood pressure tracker to use at home, blood pressure cuffs, and glucose monitors.
- We also outreach to patients due for preventative health screenings and get them scheduled for mammograms, pap smears, colonoscopies, dental sealants, annual wellness visits and immunizations.
- We conduct a text-message patient satisfaction survey monthly for our patients that is reviewed monthly. We take your feedback very seriously and determine how best to improve our care quality from these results.
