That’s why, in addition to being a Health Care Home to all of our patients, we offer AccessPlus+, a program for patients whose path to health may benefit from a more coordinated approach. The Community Health Connection
The Community Health Connection is OCHC’s outreach and education program. The Programs Director and Health Navigator Coordinators (HNCs) who are part of the Community Health Connection promote the concept of “healthy, connected communities” within OCHC and out in the community. Through the Community Health Connection program, we provide outreach, education and screenings across an array of health related topics and diseases including:

- Car Seat Classes for Ucare patients
- Dental Outreach and Education

- Flu Shot Clinics
- Employment and Job Placement Through the Goodwill-Easter Seals Partnership
- HIV/AIDS Primary Health Care, Outreach, Education, & Case Management
- MNsure Assistance
- Nutrition counseling by our in-house dietitian
